Good afternoon,

Our high school students will be taking a survey in PE classes next Monday and Tuesday (November 24 and 25) called the Youth Risk Behavior Survey.  I have attached a parent/guardian letter and a "Frequently Asked Questions" document.  

Please take a moment to read the documents.  If you prefer your child not take this survey, please complete the opt-out form and return it to the high school office by Monday, November 24 (at the latest) prior to Homeroom.
